A score form is a special type of form in SMART-TRIAL. This type of form calculates a total score for the form, based upon the answers given. The total score is calculated by summing up the values for all "included questions". This allows users to create questionnaires with rather complex calculations.
Score forms are limited to the following question types:
To create a score form:

Go to [Study Name] > Forms, or [Study Name] > Study Info.

Click + New Form.
- Enter the name of the form and click Advanced Options, then enable calculate score.
- Click Add Question. A part where you can specify the question details will appear.
- Select Question Type and specify the Question text.
- Enable, include in score for all the questions which should be included in the total score.
- Click Save Form.
For 5 level smiley, Multiple choice/Multiple response, and Yes/No question types, the user must set the 'value' of each possibility, see the image below for an example. Here possibility A will add a value of 12 to the total score
By using Calculated Questions it is possible to do more advanced calculations than sums, based upon the input of previous questions.
Only include the final calculated question to avoid the "summation" nature of the form, this way the final score will be the result of a more complex calculation.
